Important information about procedures for opening a new account.
A Living Trust is a trust that is created while a person is alive and will generally have that name in the title (e.g., John Doe Living Trust).
Signed Numerica Certificate of Trust: Link to Numerica Certificate of Trust and
EIN for the Trust (member can obtain via irs.gov) OR Trustee’s SSN/ITIN. If on hand, upload to the MRM session. If not, update the member’s email and give the member the portal link to upload: DEC e-Processing
A miscellaneous Trust is any type of trust that is not a Living Trust (e.g., Testamentary Trust, Special Needs Trust, Marital Trust, Spendthrift Trust, etc.)
Certified Copy of the court order establishing the Guardianship and/or Conservatorship, and
SSN/ITIN for the beneficiary/ward (person under the Guardianship and/or Conservatorship). If on hand, upload to the MRM session. If not, update the member’s email and give the member the portal link to upload: DEC e-Processing
SSN of the beneficiary, or
EIN of the entity accepting donations If on hand, upload to the MRM session. If not, update the member’s email and give the member the portal link to upload: DEC e-Processing
A Testamentary Trust is a trust that is generally created in a person’s will and will typically be named something along the lines of “John Doe Testamentary Trust”.
Copy of the Trust Agreement (if the Trust is created from a will, a copy of the will is acceptable), and
EIN for the Trust If on hand, upload to the MRM session. If not, update the member’s email and give the member the portal link to upload: DEC e-Processing
Letter of Appointment from either the Social Security Administration (“SSA”) or the Department of Veterans Affairs (“VA”), or
Check from either the SSA or the VA showing that the check is made payable to the RP in their role as a RP (e.g., John Doe, as Representative Payee for Jane Doe).

Certified copy of the court order directing an account to be opened, and
SSN of the person who the court ordered funds are being held for.

Certified copy of the Letters of Administration or Letters of Testamentary, and
EIN for the Estate Of note, either the deceased person or the Executor/Administrator/Personal Representative have to be a member to open an Estate account.

UTTMA stands for Uniform Transfer to Minor Account (“UTTMA”) and is an account that holds gifts or transfers that a minor has received.
